Eden was created with the formerly unmet needs of adults with disabilities aged 22 – 64 in mind. The easy first step to becoming an Eden resident is to fill out our preliminary application form.  There are several ways to apply.

You can download and print out the application and mail to the addresses below, or bring it in to us:

We accept people with physical disabilities ages 22 – 64. Eden is a lesser level of care than a nursing home or skilled care facility, so those who need the extensive levels of care, may not be eligible.

We review each completed application and conduct an interview before an applicant can be approved. Also, admission is contingent on the Division of Rehabilitation Services Determination of Need (D.O.N) screening results. Once we receive your application, we will call you to set up an interview.
